Hi all:

Does anyone have a continuity of care policy or form that you use for clients you serve who are no longer students?

Our victim advocacy services are provided only to students, and I’m looking for standards of practice you might use that allow non-students to use your services.  For us, that only revolves around a future or former student enrollment status on a limited basis (pre-enrollment, post-withdrawal, post-graduation).  Mostly, what policies, forms, and protocols do you use to decide who is eligible for such services?

Thanks in advance!  Please feel free to share on- or off-list.
