My institution is seeking best practices/standards on the following issues, and would appreciate if you could share what your institutions are doing with regard to the following issues:

1.        Mandatory trainings—do you do them, how do you document attendance/non-attendance, are there consequences for non-attendance?

2.       My campus does not have a codified system of sanctions (sanctions are determined on a case-by-case basis), but our conduct officers are seeking to determine whether our sanctions are generally in line with other institutions.  What are your campus precedents regarding sanctions for different types of sexual misconduct, and what are your campus policies regarding documentation of sanctions on student permanent records?
