We have this policy (almost exactly identical it sounds) at Randolph-Macon 
College.

Subject: University Based No Contact Directives

Good afternoon!
When two (or more) students from our campus community are involved in an
incident (sexual assault, stalking, harassment, or other issues) and one
comes forward as a victim, this student has an option to request a
university-based no contact directive.  This is a two-way notification that
is issued from our Campus Judicial Office and is in place until both
students agree otherwise.  I'm curious if other universities offer this to
students and how it functions.  Any folks willing to share?
