In an effort to minimize a survivor retelling their story, we're hoping to provide an outline of "minimal information necessary" that RA's should collect upon an initial disclosure of sexual misconduct. RA's would then pass on this information to the appropriate resource(s) who would follow up/provide a proper investigation. Does your school have any current forms that provide such an outline? This may include safety planning questions, clarification of incident/concern, need-to-know-info such as name of alleged offender, use of weapons, etc.
