Hey folks -

I'm looking for any best practices you recommend in regards to how your campus health center creates a safe, educational, and inviting space for survivors. 

Do you have any forms, website language, or even recommendations on any of the following?

1) intake form

2) discharge instruction form

3) MOU with community/campus advocate

4) Consent to deny forensic evidence exam form

5) Language on follow-up care
