Hello All, I work at a University Counseling Center. I was asked to create some guidelines for clinical documentation for clients who disclose a sexual assault. These guidelines would be especially used for counselors in training who may be questioning
how much detail to include in their session notes. I’m curious if you know of any “best practices” for clinical documentation for this area…and how would you discuss this with a counselor in training? Thanks!
