This may not be exactly what you are looking for but depending on your clients various needs Bridging Access to Care http://www.bac-ny.org/ny/index.php formerly Brooklyn Aids Task Force, may be a resource. I've copied Ian Johnson with Brooklyn Mens (K)onnect. He may also be able to point you in the right direction.
