Do any of your organizations use an internal incident reporting form, to track your student contacts and report aggregate numbers/keep statistics? Something more comprehensive than Clery forms, that include stalking and DV, involvement of drugs/alcohol, etc?

 

I would like to utilize such a form in our office to ensure that we are all keeping record of incidents in the same way and that we have access to all of them, even if someone is served by someone in the office that does not provide counseling.

 

I’m looking for something simple – checklists are great!
