Larry & others,
Dr. Louise Fitzgerald and some of her colleagues did a campus climate
survey a few years back. Her research was primarily on sexual
harassment, but the survey focused on other types of harassment as well.
I could dig up the larger report if someone would be interested in it,
but they also published their results here:

Reid, L. D., & Swan, S. (1996). Campus climate at the University of
Illinois at Urbana-Champaign: The effects of gender, race/ethnicity and
sexual orientation. University of Illinois at Urbana-Champaign,
Psychology Department.

Good luck!

Greetings Everyone,

I am looking for assistance from anyone who may have an assessment tool
that would allow me to assess our campus climate concerning violence and
safety. Please e-mail me directly with responses. Thank you
